Care Coordinator

The Wraparound Care Coordinator collaborates with community partners and clinical services to support youth and families in achieving their individualized goals through a trauma-informed, strengths-based approach. This role involves facilitating Wraparound teams, developing comprehensive care plans, and coordinating services across multiple systems such as juvenile justice and child welfare. The position requires regular travel between The Dalles and Hood River to engage clients and manage crisis situations.
