The Care Coordinator works directly with a team of Nurse Practitioners to provide intraday support and coordination that drives daily quality performance. This role partners with the clinical team to support the Chart Audit and Review program, ensuring documentation is complete and compliant with company SOPs. The position is critical to maintaining clinical quality, closing care gaps in a timely manner, and supporting positive HEDIS/STARS outcomes.
