Care Coordinator

The Care Coordinator identifies patients with preventative and chronic condition care gaps and performs patient outreach to close those gaps. This full-time role involves scheduling and tracking specialty care referrals, communicating with care teams, and working with patients to address barriers to care. The position also includes participating in quality-related workgroups and assisting with preventative and chronic care services.
