Care Coordinator

The Care Coordinator provides comprehensive mental health services, including crisis intervention, outreach, therapy, and case management, to adults with serious mental illness who are high utilizers of psychiatric and behavioral health services. The role focuses on delivering culturally responsive, recovery-oriented services with a primary emphasis on African American clients. The incumbent collaborates with multidisciplinary teams to coordinate care, advocate for client needs, and maintain accurate clinical documentation.
