Care Coordinator

The Care Coordinator partners with a Mental Health and Developmental Clinician to support families served by Child First, connecting children and families with community-based services and support. This role focuses on enhancing caregiver executive functioning skills and strengthening the caregiver-child relationship to reduce toxic stress and promote healthy child development. The position requires a reflective, relationship-based approach and collaboration with clinical teams to achieve family stabilization and optimal child outcomes.
