Care Coordinator

The Care Coordinator is responsible for the overall coordination of intensive, family-centered, community-based services for children, youth, and non-minor dependents in the Orange County Wraparound program. This position facilitates Child and Family Team meetings, guides the evolution of the Plan of Care, and assists with linkages and resource identification to promote successful home placements. The role focuses on accessing and building upon the family’s natural strengths and community resources while maintaining accurate documentation and collaborating with various community agencies.
