Care Coordinator

The Care Coordinator role involves creating, planning, and organizing a team to support youth ages 0-20 with complex behavioral health and/or substance use needs through the OhioRISE Medicaid managed care program. This position facilitates child and family team meetings to help keep families together and reduce hospitalizations while ensuring youth and families have a voice in their coordinated care. The role requires collaborating with social service providers, community partners, and family support systems to identify needs and achieve goals.
