Care Coordinator

The Care Coordinator supports the RN Care Manager and interdisciplinary teams to manage patient care in an outpatient setting, focusing on care coordination, chronic disease management, and transitions of care. This role involves conducting patient outreach, identifying barriers to care, and facilitating access to community resources to improve patient outcomes and reduce hospital utilization. The position requires strong clinical knowledge and excellent communication skills to effectively engage patients and caregivers.
