Care Coordinator

The Care Coordinator is responsible for managing chronic care for both RHC and Specialty patient populations, collaborating with healthcare providers to create a cohesive and streamlined patient experience. This role involves implementing tracking systems to identify and manage patients eligible for Care Management services, including PDCM and CoCM, to support improved outcomes. The position also focuses on coaching patients toward self-management of chronic diseases and facilitating access to appropriate medical and community resources.
