Care Coordinator

This role focuses on assisting patients with complex chronic illnesses and co-occurring psychiatric diagnoses by coordinating care, educating patients, and enhancing communication between patients and medical practitioners. The position involves collaborating with a multidisciplinary team to ensure continuity of care, promote patient adherence to care plans, and deliver evidence-based interventions to prevent disease progression. The Care Coordinator also identifies opportunities for health promotion, assesses self-care needs, and refers patients to community support services as necessary.
