Care Coordinator

The Care Coordinator serves as a liaison between clients, their families, medical and mental health practitioners, and criminal justice agencies. This role involves supporting clients by educating them on daily living activities and clinical treatment opportunities to ensure a positive treatment experience. The position requires assisting with intakes, conducting facility rounds, monitoring medications, and documenting client activities and needs. The Care Coordinator also helps clients with transportation to appointments and maintains communication among work teams.
