Care Coordinator

The Care Coordinator position at NWRC involves providing intensive care coordination and comprehensive care management to clients with ongoing chronic conditions. The role focuses on assisting clients in making the best use of current services and supports, while helping them find additional beneficial resources. Care Coordinators work primarily in the field, coordinating medical and social services and facilitating communication between clients, their medical providers, family, and other caregivers.
