Care Gap Program Coordinator

The Care Gap Program Coordinator supports the Health System Care Gap team in managing and enhancing gap closure programs for health system customers. This role involves working with large, complex data sets to ensure accurate program setup, patient eligibility verification, and downstream operational readiness. The coordinator partners with internal teams to intake, validate, and coordinate new care gap program orders, analyzing customer-submitted files and identifying data quality issues. Additionally, the role takes on sole ownership of specific responsibilities that support the execution and tracking of individual care gap programs.
