Care Management Coordinator

This role assists with and coordinates care management activities under the direction of a licensed nurse or social worker, functioning as a team member in member assessment, planning, and advocacy. The position involves collaborating with providers and community resources to ensure appropriate care coordination and support for members' medical, behavioral, and social needs. It requires documenting the care management process, monitoring clinical outcomes, and providing behavioral health support to members in crisis.
