Care Management

This role is responsible for care coordination and transitions planning throughout the acute care patient experience, working collaboratively with physicians, nurses, and other healthcare professionals to ensure timely progression to the appropriate level of care. The position aims to prevent unnecessary admissions or readmissions by facilitating effective resource coordination and communication across the continuum of care. The Care Management process addresses complex clinical and social situations efficiently to optimize patient health and resource utilization.
