Care Management

This role is responsible for care coordination and transitions planning throughout the acute care patient experience, working collaboratively with physicians, nurses, and other healthcare professionals to ensure timely patient progression. The position focuses on preventing unnecessary admissions or readmissions by facilitating effective resource coordination and communication across the care continuum. The Care Management process addresses complex clinical and social situations to optimize health outcomes and appropriate resource utilization.
