Care Manager I - Waiver

The Care Manager I - Waiver provides integrated, whole‑person, person‑centered care management for individuals and families with special health care needs, coordinating physical health, behavioral health, pharmacy, and other resources. The role conducts comprehensive assessments, develops and monitors plans of care, and collaborates with members, providers, and community partners to ensure appropriate services and reduce unnecessary hospital and emergency use. This full‑time remote position requires residence in North Carolina and occasional travel for onsite meetings and member visits.
