About this role
The Care Manager I provides case management services for a caseload of 17–22 Community Living Fund clients, including older adults and adults with disabilities. The role focuses on assessing clients’ medical and psychosocial needs and developing and monitoring client-centered community living plans to ensure quality care and safe aging in place.
