Care Manager II

The Care Manager II supports aging adults and adults with disabilities by assessing complex medical and psychosocial needs and developing client-centered community living plans. The role focuses on monitoring and coordinating services to maintain clients’ health, independence, and quality of life while supporting IOA’s aging-in-place mission. It contributes to program quality improvement and participates in team and community collaboration.
