Care Manager Lead

The Care Manager Lead provides case oversight, administrative, and operational leadership for the Enhanced Care Management (ECM) team while maintaining a caseload of approximately 25 ECM members. This position ensures high-quality, member-centered care coordination consistent with CalAIM ECM requirements, Sacramento Children's Home policies, and contracted Managed Care Plan expectations. The role promotes staff development, quality improvement, and program performance through supervision, documentation review, and collaboration with internal and external partners.
