Care Manager RN - Administrative Services

The Care Manager RN is an integral member of the CHAT or ACO team, responsible for evaluating and managing the changing health and service needs of patients. This role provides optimum individual care for the patient/family unit using the nursing process and interdisciplinary team members. The position involves collaborating with the patient's Primary Care Physician and other health team members to create and implement a written care plan.
