Care Manager-Youth Program

The Care Manager-Youth Program provides comprehensive case management for at-risk and unhoused youth, supporting their education, employment, and housing needs through the McKinney‑Vento process and coordination with community resources. The role uses a trauma‑informed, person‑centered approach to help youth obtain vital documents, access mental health and medical services, and achieve stability and self‑sufficiency.
