Care Manager

Liberty POST is seeking bilingual Care Managers to provide direct services for children in early intervention, special education, and private programs across multiple locations in New York. The role involves outreach, eligibility assessment, developing and coordinating individualized Plans of Health Care, and delivering at least two health home services per month, including face‑to‑face visits. Candidates must have a BA and relevant experience working with children and families in health or social service settings.
