Care Manager

The Care Manager provides comprehensive health home services to Medicaid-enrolled children, youth, and adults with complex health needs, delivering care coordination, health promotion, transitional care, and family support. The role involves conducting assessments, developing person‑centered plans, collaborating with providers and community partners, and meeting productivity and quality benchmarks while traveling throughout Suffolk, Nassau, and select NYC boroughs.
