Care Manager

This role is responsible for care coordination and transitions planning throughout the acute care patient experience, working collaboratively with physicians, nurses, and other healthcare professionals to ensure timely patient progression and prevent unnecessary admissions. The position involves facilitating care across the continuum through effective resource coordination while balancing optimal health outcomes with patient self-determination. The Care Manager also addresses complex clinical and social situations efficiently, particularly in the Emergency Department, to avoid unnecessary hospitalizations.
