Care Manager

This role provides support for care management and coordination of long-term services and supports (LTSS) for members with high-need potential. The Care Manager collaborates with a multidisciplinary team to ensure integrated delivery of member care, assesses member needs, and develops comprehensive care plans to achieve desired health outcomes. The position involves conducting assessments, authorizing waiver services, and addressing barriers to care while promoting continuity of care across behavioral health and community resources.
