Care Manager

This role provides support for care management and coordination of long-term services and supports (LTSS) for members with high-need potential. The Care Manager collaborates with a multidisciplinary team to deliver integrated care, conduct comprehensive assessments, and develop individualized care plans to achieve desired health outcomes. The position involves ongoing monitoring of care effectiveness, facilitating interdisciplinary meetings, and addressing barriers to care to ensure quality and cost-effective member services.
