About this role
The Care Navigator is a per diem member of the Behavioral Health team at Clifton Springs Hospital who supports integration between primary care and behavioral health providers. The role focuses on coordinating mental health care and referrals within and outside the clinic and may involve providing or facilitating evidence-based treatments. The position reports to the Manager, Behavioral Health/Primary Care Integration.
