About this role
The Care Navigator is a member of an Integrated Care team supporting coordination of mental health services within a primary care clinic. The role connects patients, primary care providers, behavioral health clinicians, and psychiatric providers, and coordinates referrals to internal and external services. The position may deliver brief evidence-based interventions and supports measurement-based tracking of patient outcomes. The Care Navigator reports to the Manager, Behavioral Health/Primary Care Integration.
