About this role
The Care Navigator is a member of an Integrated Care team supporting behavioral health within a primary care clinic. The role coordinates mental health care and referrals both inside and outside the clinic and may deliver brief, evidence-based treatments. The position reports to the Manager, Behavioral Health/Primary Care Integration and supports collaboration among PCPs, BHCs, and psychiatric providers.
