Care Navigator Level 2

NorthCare is seeking a Care Navigator Level 2 to join their Assertive Community Treatment team, providing community-based mental health services and case management to individuals with severe mental illness. The role involves meeting clients in various settings to coordinate care, assist with life goals, and offer wellness coaching and crisis support. This position requires a high degree of autonomy and flexibility to deliver over 50% of services outside of traditional clinic environments.
