About this role
The Care Navigator is a member of an Integrated Care team who coordinates and supports mental health care within the clinic and arranges referrals to appropriate services. The role involves working with primary care, behavioral health, and psychiatric providers and may include delivering evidence-based brief treatments. The position reports to the Manager, Behavioral Health/Primary Care Integration and operates within the inpatient psychiatric setting at Clifton Springs Hospital.
