Care Navigator

The Care Navigator serves as a liaison between KSOSN, partner facilities, dialysis organizations, and transplant teams to assist patients with optimal dialysis starts, transplant list placement, and comprehensive care education. This role focuses on creating a quality environment, promoting patient safety and experience, and supporting the clinical team. The position partners with leadership to achieve organizational goals while managing patient referrals and admissions.
