Care Navigator

The Care Navigator role at Southwest Behavioral Care, Inc. provides individualized and community-based services to assist clients in achieving long-term sobriety and recovery goals. This position involves connecting clients with essential services, educating them and their families about the recovery process, and offering goal-oriented encouragement and support. The role is part of the Center of Excellence, a 24/7 access program designed to proactively address the opiate epidemic through a multidisciplinary team approach.
