Care Partner

3Bridges is seeking an experienced Care Partner to join their Aged Care team on a full-time, fixed-term parental leave contract based in Carss Park, New South Wales. In this role, you will manage an allocated caseload of older clients, building trusted relationships with clients, families, and carers while developing and reviewing individual care plans. The position involves coordinating services, responding to changing client needs, and ensuring person-centred support that promotes independence, safety, and wellbeing.
