Case Aide

The Case Aide assists families in meeting concrete needs to increase safety, reduce risk, and promote wellbeing using the Family Connections model of practice. This role involves collaborating with a team to provide assessment, goal setting, and case management to prevent foster care. The position requires conducting home and office visits, providing outreach and referrals, and supporting families with applications and documentation.
