Case Management Assistant

The Case Management Assistant provides hands-on post-acute care planning services to hospitalized and emergency department patients under the direction of RN or Social Work Case Managers. The role involves coordinating post-acute care arrangements, managing medical record transfers, and communicating with facilities to ensure compliance with regulations. This position serves as a liaison between case management staff, vendors, patients, and families to facilitate smooth transitions of care.
