Case Management Coordinator

Saint Clare's Health is seeking a Case Management Coordinator to assist with the coordination of activities within the Case Management Department under the direction of assigned Case Managers. The role involves supporting the development and implementation of safe, patient-focused discharge plans by interviewing patients and families to gather necessary data. Additionally, the coordinator assists with case management referrals, authorizations, and the maintenance of Medicaid state submission forms.
