Case Management Nurse, Senior

The Case Management Nurse, Senior will report to the Manager, Care Management and collaborate with clinical teams to determine, develop, and implement care plans based on accurate member assessments. This role focuses on promoting quality of care, cost-effective services, and continuous improvement while managing complex cases including chronic conditions and pre-natal care. The position requires a current RN license, an Associate's Degree in Nursing, and at least five years of prior healthcare experience, with prior transplant experience being required.
