Case Management Support

The role provides a wide range of support services to the Case Management staff, assisting RN Case Managers and Social Workers with discharge planning, insurance communication, and data collection. It involves coordinating patient care, arranging post‑acute services, and following patients post‑discharge to reduce readmissions, while maintaining thorough documentation and customer service.
