Case Management

This full-time position supports individuals with Intellectual and/or Developmental Disabilities in developing and monitoring their personal goals using a person-centered approach. The role involves coordinating safe and effective services, conducting regular visits, and facilitating meetings to ensure comprehensive care. The position requires maintaining accurate documentation, communicating with stakeholders, and managing a caseload of approximately 30 individuals.
