Case Management

This position is responsible for monitoring a caseload of clients in a short-term life management program, establishing goals, and evaluating progress through routine meetings. The role involves serving as an advocate to help clients acquire necessary services and functioning effectively in their environment. Key duties include conducting eligibility interviews, performing needs assessments, and maintaining accurate case records in the Homeless Management Information System (HMIS).
