Case Management

This role involves monitoring a caseload of clients in an emergency day shelter program, establishing individualized program plans, and evaluating their progress through routine meetings. The Case Manager serves as an advocate to help clients acquire necessary services and ensures compliance with funding requirements. The position also includes assisting with the onsite food pantry and maintaining accurate case records.
