Case Manager 1

Volunteers of America Colorado is seeking a Case Manager 1 to provide community-based case management and care coordination services for Veterans, service members, and their families participating in the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program. The role involves conducting outreach, performing needs-based assessments, and connecting participants to VA and community resources to address behavioral health needs and social determinants of health. This position focuses on promoting participant stability, recovery, and overall well-being using trauma-informed and recovery-oriented practices.
