About this role
Presbyterian Healthcare Services is hiring an RN Case Manager for the Employee Health Clinic at Northside in Albuquerque. The RN Case Manager independently coordinates and facilitates care for assigned populations through assessment, care planning, implementation, monitoring and evaluation to achieve cost-effective, quality outcomes. The role supports members and care teams across clinical and community settings and uses electronic charting systems.
