The Case Manager provides a range of services for clients with psychiatric and/or substance abuse issues, including advocacy, outreach, discharge coordination, and service linkage. This role works in coordination with interdisciplinary teams within the Adult Partial Hospitalization Program to help clients access entitlement services and participate in their own care and recovery.
