Case Manager (CPST)

The Case Manager (CPST) provides community-based services to clients with severe and persistent mental illness, working with multidisciplinary teams to assist in meeting recovery goals. The role involves assessing client needs and linking them to essential community resources such as food, shelter, and employment. Services are delivered individually or in groups, either face-to-face or by telephone, in locations that best meet client needs.
