Case Manager I (ICMS)

This position serves as part of a permanent supportive housing case management team, providing intensive case management services to formerly homeless individuals and families with chronic medical conditions and co-occurring disorders. The role utilizes frameworks like Motivation Interviewing and Critical Time Intervention to support tenants in achieving their goals through comprehensive services including intake, assessment, counseling, and benefit assistance. The Case Manager I is responsible for documenting interactions in specialized databases and presenting case reviews to supervisors.
