Case Manager I/II

This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet consumer needs, and serving as a Qualified Intellectual Disability Professional (QIDP). The position requires monthly monitoring of programs and compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
