Case Manager - Intake

Developmental Pathways is seeking an Intake Case Manager to join their Systems Navigation Team, providing compassionate, person-centered service and support coordination to help individuals access Medicaid Waiver and State General Fund services. The role involves conducting in-home assessments, engaging with families and community providers, and collaborating with team members to enhance service access. This hybrid position requires frequent travel within Arapahoe, Douglas, and Elbert counties for screenings and assessments, alongside remote and office-based work.
